General information
It is always necessary to submit official CZU forms that you will find in the Documents a Forms for Study Stays section.
At the very end of your stay (no sooner than during the last three days of your stay) ask your coordinators abroad to sign the Confirmation of the study period. Do not forget that the confirmation must contain exactly the same data as your Financial agreement (even if only for one day, a change to the duration of the stay may cause problems, and you may also have to return an adequate part of the grant for every missing day!).
After the end of your study stay, your host university will complete a Transcript of Records. This is an overview of studied courses along with grades and the number of credits. Some universities issue their own forms, some use official forms, and some will send you transcripts by email or send them directly to CZU IRO. Subjects in the Transcript of Records must match the Online Learning Agreement, including potential Changes to the Learning Agreement. If you do not pass one of your exams during the semester, instead of the grade, you will need to attach to the Transcript your attendance confirmation in the course during the semester, which will be confirmed by your teacher (there is no official form; however, the confirmation must include the name of the student, course, its duration and signature of the teacher). After you return, it will depend on your home faculty if it considers the number of ECTS credits acquired during your stay sufficient.
Another important form is the Participant Report, which must be filled in. The report is filled in online, after your stay is over, you will receive an automatic e-mail from eu-corporate-notification-system@ec.europa.eu / subject: Erasmus+ participant report, with a link to where you need to fill in the report).
You must take all of these forms - ORIGINALS – to the International Relations Office of the CZU Rectorate (Ing. Adam Vacek or Mgr. Radka Kalousová, outgoing@czu.cz) within 14 days after the end of your study stay.
Return to the faculty
After you return, on the basis of submitting a Transcript of Records, at your home faculty you will make sure your courses are recognized. You must then fill in the Learning Agreement – Table C – outcomes of subjects from abroad. If the foreign university confirms your outcomes on its own form, Table E does not have to be confirmed by the foreign university. You will then fill in the last table of the Learning Agreement – Table D – Recognition of Outcomes, which will contain all of the courses recognized according to the Transcript of Records. Completed courses will be assigned to the courses taught at CZU, or recognized as electives, and the Table D document will be confirmed by the faculty. Submit the original copy for filing at the CZU International Relations Office at the latest within 2 months after the end of your foreign study stay.
Documents that you need to submit
The following is a list of documents that you must submit within 14 days after the end of your stay:
- Online Learning Agreement - introductory part - before mobility with courses before departure, including changes, if applicable
- Transcript of Records
- Confirmation of study period - the original signature is required!
- Final Report (online)
